Overview

In his latest book, Dr. Robert A. Norman introduces us to the intriguing concept of preventive dermatology. Although dermatologists have long been patient advocates and have stressed vigorously on the importance of sun avoidance and protection, there is still much more that we can do to prevent disease. Dr. Norman and his skilled coterie of collaborators discuss two distinct types of prevention in dermatology: the prevention of skin diseases and the prevention of systemic disorders, some with only very indirect connections to the skin. The frst is fairly well known to dermatologists; the second is truly an emerging concept of great importance. Educational efforts to prevent or at least control skin disease may range from the proper use of sunscreens to weight loss in psoriatic patients, the avoidance of trigger factors in rosacea, proper skin care in atopic dermatitis, or adoption of a low-fat diet to decrease the incidence of actinic keratosis and nonmelanoma skin cancer. Another good example is the use of vaccines to protect against diseases such as herpes zoster and genital HPV infection in females.

Description: A preventive approach to treating dermatological conditions obviously has overlap with established therapies and both are discussed in this book. The beneficial roles of the proper moisturizers, syndets for cleansing, vaccines to prevent infections, proper nutrition, and vitamin supplementation in maintaining healthy skin are discussed.
Purpose: The purpose is to educate dermatologists on how patients can be more proactive in taking care of their skin, especially if they have a genetic tendency for a particular disorder. The book summarizes a global approach to minimizing disease flares. Many chapters are devoted to the presentation and immunologic mechanisms of diseases such as bullous disorders.
Audience: The audience is dermatologists.
Features: The book presents a number of practical tips to avoid the development of problems such as skin infection, nail and foot fungus, and fetal eczema precautions for the pregnant mother. This information from the shaded summary tables could easily be incorporated into a useful patient handout. The information on cleansing and moisturizing is repeatedly emphasized in multiple chapters by different authors. The index is a little spotty. I could not find some drugs in the index that I had read about in the book, such as cyclosporine or mycophenolate mofetil. I finally located them under psoriasis, where they were listed in a subcategory. The appendix lists websites to which patients may be referred for additional information. Tips on prevention are listed for various skin ailments.
Assessment: This book presents a general treatment overview of common skin disorders and suggests precautions to take to decrease complications and exacerbations. For a more extensive discussion of therapies, readers are referred to references such as Evidence-Based Dermatology, 2nd edition, Williams et al. (Wiley-Blackwell, 2008), Comprehensive Dermatologic Drug Therapy, 2nd edition, Wolverton et al. (Elsevier, 2007), and Treatment of Skin Disease: Comprehensive Therapeutic Strategies, 3rd edition, Lebwohl et al. (Elsevier, 2010).
