Overview

It is now 11 years since the publication of the first edition of Normal and Abnormal Swallowing: Imaging in Diagnosis and Therapy. These 11 years have seen an enormous change in the specialty of the study of dysphagia. First, we have seen the tragic loss of two of the pioneering giants in the field: Wylie J. Dodds, MD, of the Medical College of Wisconsin, Milwaukee, and Martin W. Donner, MD, of The lohns Hopkins University School of Medicine, Baltimore, Maryland, a former coeditor of this book. Second, there has been a steady expansion in the number of professionals interested in and working in this spe­ cialty. The last 11 years have also seen the steady growth of the multidiscipli­ nary journal Dysphagia, devoted to swallowing and its disorders, as well as the formation of the Dysphagia Research Society, which held its tenth annual meeting in October 2001. The dysphagia special interest division (SID 13) of the American Speech and Hearing Association (ASHA) now has some 3,000 members. At the same time, the world population is aging. Dysphagia will be an important health issue in this aging population.

Description: This book covers the radiographic assessment of dysphagia, principally with the use of barium contrast fluoroscopy. A brief introduction to normal swallowing physiology, as well as the phases and stages of swallowing, is provided. The last edition was published in 1991, and the newer edition provides some overview of newer techniques to assess dysphagia.
Purpose: The purpose is to familiarize the reader with how the spectrum of swallowing disorders is assessed with imaging of the pharynx, and to describe abnormal findings on radiographic assessment. These are worthy objectives since the incidence of some degree of dysphagia is rising rapidly with the aging population and advances in medical and surgical treatment of other advanced diseases. The book meets the author's objectives with regard to the spectrum of radiographic findings, although an expanded section on nonimaging aspects of dysphagia (including pathophysiology, epidemiology and prognosis) would make this book more accessible to otolaryngologists.
Audience: The book is written for practitioners in gastrointestinal radiology and speech/language/swallowing pathology, and serves as a useful handbook for most of the abnormalities that may be expected during modified barium swallow examinations. Practicing otolaryngologists may also benefit from a greater understanding of the spectrum of swallowing disorders and aspiration risk, particularly pertaining to postoperative head and neck cancer patients. The editor and authors are appropriate authorities in this area.
Features: The book covers details of an appropriate oral contrast fluoroscopic examination of swallowing, and spends the bulk of the text reviewing abnormalities on this examination. These abnormalities are presented with appropriate examples, and are put into perspective with regard to normal and abnormal swallowing physiology. Newer methods to assess swallowing and dysphagia are briefly presented, including the use of ultrasound, computed tomography and magnetic resonance. The shortcoming of this book is the relatively narrow audience of those practitioners involved with "hands-on" radiographic assessment of dysphagia.
Assessment: The quality of this book is overall very good, serving as a compendium of contrast radiographic findings in normal and abnormal swallowing. It is particularly useful to the gastrointestinal radiologist and speech/language/swallowing pathologist. This book introduces some newer techniques that have been developed in the 11 years since the last edition.
