Overview

This book is aimed at broad spectrum of intensive and coronary care personnel and provides rapid access to important information on the treatment of the patient in the coronary care unit (CUU), in an easily readable and accessible format. It comprises a series of articles on key aspects of emergency care of the cardiac patient and reviews some of the most advanced concepts in the CCU. Whilst assuming a basic knowledge of underlying problems, the book nonetheless outlines key physiological principles where necessary, and critically reviews current literature and best practice. The book focuses on providing key practical information on treatment techniques for busy CUU personnel and features some important topics such as shock, sever sepsis, echocardiography, catheter-based treatments for acute cardiac emergencies, cardiac arrest and CPR, cardiogenic shock, heart failure and the role of surgical intervention.

This multiedited text is written by staff physicians at the Blackpool Victoria Hospital, Departments of Anesthesia, Intensive Care Unit, Cardiology, and Cardiac Surgery. It is divided into two general sections: Basic Principles, such as cardiac physiology and pharmacology, and Specific Problems, such as angina, cardiogenic shock, and heart failure. The purpose is to review all modern therapy for cardiac emergencies in a very brief fashion. This is not designed as a substitute for general cardiology text books; the purpose is to make information easily accessible. The book is primarily written for house officers, trainees, and perhaps nurses working in medical specialties. This text is full of bullet points on various cardiac medical problems. The idea is to outline treatment in a very brief fashion. There is a good discussion of preload, afterload, and contractility. There is a nice chapter on right ventricular function, as well as a chapter on mechanical aids to left ventricular function (i.e., intra-aortic balloon pump, pacemakers, and intra-cardiac defibrillators). There is even a short section on pacemaker interference. Although intended for trainees, there is a section on angioplasties and stents, as well as aortic stents and valvulae plastics. The chapter on heart failure is interesting. Some authors might disagree with the concept that the use of digoxin in sinus rhythm is debatable, because it only keeps patients out of the hospital and allows them more activity, but has no effect on survival. The use of the pulmonary artery flotation catheter is described with a brief discussion on complications. An American audience will find that some of the drugs mentioned are not availablein the U.S. (Nicorandil, Enoximone, and Dopexamine). All handbooks suffer from the lack of the information that they must, by definition, leave out. These authors do a credible job of including basic material in a bullet point form that would be very helpful for house officers seeking some good information in a very brief period of time. Nurses would also find this a worthwhile text, especially if they are intensive care unit nurses. Although authorship of the chapters individually are not identified, the text is a welcome addition to the material available for trainees and house officers.
