Overview

The twenty-first century will be the era of the market economy, a century char­ acterized by the penetration of market forces into every social field, even into government activities, education, and medical care. Guidelines for the provision of clinical care have been developed in recent years chiefly in American health-care services, which are the most thoroughly exposed to the market economy. The problem of escalating medical costs in the United States led the government and consumer groups to introduce clinical practice guidelines. Guidelines were introduced to control medical costs and quality. Initially, guidelines were developed mainly on the consumer side, but professionals, too, soon recognized the importance of clinical practice guidelines. The involvement of diverse groups in the development of guidelines has inten­ sified the need to create and improve scientific methods for drawing up those guidelines. Owing to the development of systematic and structured abstraction methods, evidence-based guidelines have been proposed. Principally, guidelines are a sum­ mary of published treatments created by statistical analysis of clinical outcomes.

Description: This is a compilation of practice guidelines addressing several common urological disorders. Background material with appropriate references are cited and the authors have detailed their approaches in evaluation and management based on the information presented. The book is unique in that it is an attempt to address an important evolving issue in clinical medicine today, the development of practice guidelines to assist the physician in the management of patients presenting with specific disorders.
Purpose: The purpose is to present background information on the concept of practice guidelines and outline their use in specific urologic disorders. Authors also address important issues including the evaluation and ethics of new interventions and the method of evaluating and developing practice guidelines. In the preface it is stated "Guidelines for clinical practice, developed from evidence-based medicine by a panel of physicians, surgeons, and other medical professionals, clearly can lead to better diagnostic procedures and treatment modalities for patients." These are worthy objectives and it is important to bring them to the medical profession. Most certainly these objectives have been met.
Audience: The book was written for practicing urologists. Others who are interested in guideline development and application may also find the book of value. Urologists in training will also find valuable information in the book. The editor and authors are well respected authorities in medicine and, specifically, urology.
Features: The book is composed of 10 chapters and all but two have multiple authors. Clinical topics reviewed include transurethral resection of the prostate, benign prostatic hyperplasia, ureteral stones, staghorn calculi, ureteropelvic junction obstruction and female stress urinary incontinence. Other chapters address guideline development and evaluation and issues related to new intervention development. The clinical topics are common urological disorders and the others address important and much debated issues. All are reviewed in detail and are appropriately referenced. Controversy and differences in opinion and practice are also appropriately addressed. Shortcomings are limited, but relate to the omission of other important topics such as carcinoma of the prostate, malignancies of the kidney and bladder, male infertility and erectile dysfunction.
Assessment: This book is excellent and should be of interest to all practicing urologists and those in training. Important issues are reviewed. Guidelines are becoming increasingly important and though many of the topics have been addressed in other publications, the compilation as presented by the editor is of value. The book should serve as a ready reference to the busy practitioner.
