Description: This book describes open and minimally invasive resections performed for carcinoma of the esophagus, stomach and duodenum, small intestine, and colorectum and it includes pancreaticoduodenectomy. Procedures are illustrated with color photographs, line drawings, and (for some chapters) excellent videos are accessible at an accompanying website.
Purpose: The book is "designed to provide a step-by-step surgical approach, highlighting key learning points and potential operative pitfalls." It largely fulfills these worthy objectives. The book thus joins and should be compared to a variety of operative texts and atlases, some devoted to oncologic procedures, but others that are more general in scope. This is a concise and brief compendium of many techniques in this chosen field of surgery for gastrointestinal tumors. In this area, it may prove a useful resource.
Audience: The authors suggest that this book will be valuable to surgeons at "all levels of training," but the brief format assumes a certain level of familiarity that makes it more appropriate to more advanced learners such as surgical oncology fellows and/or fully trained surgeons. The editor and chapter authors are all highly credible authorities in their fields.
Features: The book is nicely organized in the usual anatomic fashion, starting with the proximal gastrointestinal tract and progressing distally. The inclusion of some specialized procedures, such as transanal resection, is highly valuable. Any book that covers 28 procedures in about 300 pages will be a little brief in places, and this is true here. In addition, adherence to a fixed chapter format means that some chapters seem short and others baffling long or even unneeded. Thus the chapter on open pancreaticoduodenectomy could definitely include more technical details of this difficult procedure (and maybe some information on pylorus-preservation). The chapters on open partial and total gastrectomy give few details on how to perform an adequate lymphadenectomy, surely one of the hallmarks of excellent cancer surgery in this region. Similarly, the chapter on open small bowel resection includes the formulaic illustration of patient position — surely not needed for the intended audience — but omits any technical hints on lymphadenectomy, and devotes its few illustrations to standard anastomotic techniques. Many of the color photographs and even some the art on close inspection are blurry or unclear. Most of the black-and-white line art is nicely reproduced, but some of that is a bit blurry as well. Less space devoted to patient positioning and more to details of operative procedures would have been welcome. Excellent videos supplement the text for some procedures, and where the book is good, it is quite good indeed.
Assessment: The best parts of this book are the accompanying videos, which are available primarily for the minimally invasive procedures. As a book, it is unfortunately not as complete or clear in coverage as other more comprehensive ones, particularly in its coverage of open surgical procedures.
