About the Author

Daryl I. Smith, MD, Associate Professor, Department of Anesthesiology, University of Rochester, School of Medicine & Dentistry. He is also Division Chief for the Acute Pain Division with extensive experience in performing peripheral blocks and neuraxial anesthesia.

Nobuyuki Hai Tran, MD, is Assistant Professor in Pediatric Anesthesiology, Faculty of the Pediatric Pain Division, Faculty of the Adult Acute Pain Division, University of Rochester, Department of Anesthesiology and Pain Management. His fields of interest include myopathy and malignant hyperthermia and complex syndromic airway mangement.

Description: This book is a summary collection of neuropathic pain disorders and their known molecular biological bases. The book is a first edition and represents the first of its kind to accumulate current clinical and biochemical information about neuropathic disorders into one text. There are accompanying illustrations and figures to show signaling pathways and molecular structure.
Purpose: This book aims both to provide a comprehensive source on the pathogenic origins of neuropathic disorders with a focus on the molecular bases and to distinguish disorders with diverse mechanisms from each other. This is a worthy objective in a clinical environment in which physicians of all specialties are asked to see patients with neuropathic pain without necessarily having the background knowledge of specific disorders.
Audience: The intended audience is twofold. There is a clear focus on molecular biology and signaling pathways, which is intended for basic science and translational researchers. There is a significant amount of content with a clinical focus, which is intended for those treating these conditions. Much of the book is fairly technical and may not be relevant to clinicians simply looking for a treatment for a given patient's condition, but will be of interest to clinicians wishing to understand the pathogenesis of a patient's neuropathy.
Features: This book is broken into two parts: the first is five chapters on the mechanisms behind chronification of acute pain and the second is a discussion of various mechanistically distinct neuropathic syndromes. Part 1 delves into the signaling pathway and molecular mechanisms that contribute to chronification of acute pain (e.g. the role that TNF-alpha plays in sensitizing nociceptors and the currently characterized signaling pathways that contribute). For these discussions, the accompanying illustrations help break up the text and provide a visual reference for a technical discussion of genetic and molecular pathways. The chapters of the first section include discussions of research into therapeutics and clinical application of the knowledge about these mechanisms. Part 2 is more clinically focused, with chapters on distinct neuropathic pain disorders. The chapters begin with a summary of the syndrome and the known mechanisms behind, for example, diabetic neuropathy or pressure induced neuropathies, and end with discussions of specific treatments for each condition. It includes technical discussions of signaling pathways and molecular mechanisms on par with reading review articles written for basic scientists as well as summary and clinically focused sections more appropriate for general clinicians. The book is more suited for those already involved in neuropathic pain research or those with a good background in molecular biology and a particular interest in treating neuropathic pain than it is for a primary care provider only interested in what to do for a specific patient, but nevertheless, the book provides an educational resource for a motivated, relative novice that has not previously been available.
