Overview

The spectrum of unstable coronary syndromes has been the object of steadily increasing research particularly in respect of novel diagnostic and treatment modalities. Although the WHO criteria for the diagnosis of acute myocardial infarction have been known for decades, there is still much debate as to the proper use of these, especially the handling of biochemical markers. Traditional enzymes have limitations in diagnostic power and new markers have unclarified applicability with regard to standardisation of assays and decision limits. Furthermore, the growing understanding of the conception of minimal myocardial damage in the borderzone between unstable angina pectoris and myocardial infarction necessitates refinement in the definitions of the various entities within the kaleidoscope of acute myocardial ischaemia. There has been a strong impetus within the European Society of Cardiology to present this subject to a broad audience of cardiologists, and as a consequence the theme of biochemical markers has become an important constituent of the Education and Training Programmes of the Society. This text focuses on biochemical, clinical, economic, and pre-analytical considerations in the use of markers of myocardial injury for the early assessment of cardiac damage. The purpose is to address key issues in the utility of new biochemical markers that can be used for the early assessment of myocardial injury. The editors aim to address clinical, economic, and pharmacologic considerations as well as the considerations in the use of each currently utilized marker. The intended audience is biochemists, pharmacologists, and general physicians as well as clinical and basic researchers. This text assumes a required base of knowledge in this area. Therefore, the primary target audience would be clinical researchers and biochemists with a secondary audience of pharmacologists and general physicians. This text covers clinical and biochemical issues relating to the utility of each new marker discussed for assessment of myocardial injury as well as addressing some of the fundamental pathophysiologic mechanisms which underlie this clinical problem. While there are not unique features to this text, selected subjects addressed include economic considerations in the use of these markers as well as their evaluation in clinical trials and the pre-analytical (pre-clinical) considerations which must be taken into account in using and interpreting marker levels. This text provides current information -- clinical, biochemical, pharmacologic, and economic -- on the use of biochemical markers of myocardial injury. The overall quality of this text is average and each chapter is relatively brief. However, there are not a lot of comparative texts that are published specifically on these topics, sothis text will be of some interest to biochemists, clinical researchers, and physicians. The editors have touched on several key topics in this area and each chapter is adequately referenced. While it is a worthy addition to the field, this text provides neither the depth of information nor the structured approach of another recent text with certain similarities, Wu's Cardiac Markers (Humana Press, 1998).
