Description: This is the second volume in a series exploring diagnostic principles and management strategies central to the care of solid tumor patients, from a system-based or organ-based perspective.
Purpose: This book explores the natural history, diagnosis, and standard treatments for lung cancers. The editors aim to present the fundamentals of lung cancer management in a succinct, readable format useful for clinical reference. These objectives are worthy, as there are few books available with this narrow a scope.
Audience: The editors have assembled a number of international experts to explore a wide variety of lung cancer-specific topics. They intend this book for general oncology practitioners and other interested healthcare professionals.
Features: The book is divided into three logical sections: first, recommended evaluation for a patient with suspected disease; next, standard treatments of lung cancer patients according to stage; and lastly, a description of special circumstances, including treatment of the elderly and palliative care options. A number of broad concepts central to the care of solid tumor cancer patients are emphasized (no radiographic modality is sufficient to take the place of tissue to confirm a diagnosis), and areas of current debate are not ignored (optimal management of IIIA-N2 NSCLC). In fact, the introduction of controversial topics using a point-counterpoint delivery is among the most effective methods the book uses to present the material. While the authors mention both personalized treatments and targeted therapies, insufficient attention is paid to EGFR-mutant lung cancer. Based on recent phase III IPASS data, mutations in EGFR are not merely "another promising therapeutic target," but a marker predictive of response to EGFR-TKI therapies. As a result, EGFR and KRAS mutation testing of patients with newly diagnosed metastatic disease is increasingly performed upfront in most large cancer centers in this country. The predictive (and prognostic) importance of these mutations should have been emphasized in a way that separates these mutations from the list of hypothetical targets for which only investigational agents exist in the clinical trial setting.
Assessment: Overall, the editors have assembled a useful reference that reviews the concepts essential to lung cancer management. As oncology continues to evolve into a number of disease subspecialties, resources like this will be increasingly valuable to all healthcare professionals involved in the care of cancer patients.
