Description: This book is a resource for nephrology in the ICU setting, particularly in the case of acute kidney injury (AKI). A thorough review of the most recent literature in renal replacement therapy (RRT) is presented, and generalized chapters cover acid-base abnormalities, electrolyte abnormalities, water homeostasis, and nutrition in the ICU setting.
Purpose: The primary focus of this book is on AKI under circumstances encountered in the ICU. Using an extremely robust library of the most recent medical literature, strategies are provided for the prevention of AKI in specific circumstances, management strategies for when AKI is encountered, and a comprehensive review of RRT and goals. The latter aspect is currently a topic of hot debate in the literature, with many yet-unanswered questions; consequently a single reference point to discuss these points has been much needed.
Audience: This is an excellent resource for anyone involved in critical care medicine, whether nephrologists, pulmonologists, or anesthesiologists. Given the high prevalence of AKI in the ICU population, all of the topics will be encountered on a daily basis.
Features: The strongest topics with the most evidence-based reviews are those coving RRT, both intermittent hemodialysis and continuous renal replacement therapy. A vast collection of resources has been pooled to help guide timing of initiation, mode of RRT, dosing goals and anticoagulation schemes. A close second deals with the prevention of AKI in specific scenarios, again covering the most current medical literature to date. The chapters regarding water homeostasis, acid-base and electrolyte abnormalities, while comprehensive, are not unique and consequently should not be viewed as cutting-edge. Smaller chapters are geared towards pediatric patients, shock, respiratory failure, and multiple organ dysfunction, but again provide merely a brief review and are not for intensive study.
Assessment: This book provides one of the strongest reviews of the management of patients with AKI, particularly with RRT, currently available. It is easy to read and graphs and charts are provided when necessary. This book does not belong on the shelf of every clinician involved in critical care; the topics are encountered so frequently that the book will not have a chance to be shelved.
