Overview

This book is a complete guide to the diagnosis and management of any infectious disease which may affect the mother or the fetus during pregnancy. Pregnancy is a unique condition in which the interplay of endocrine and immune influences leads to altered severity and susceptibility to infectious diseases. These infections, in turn, are a substantial cause of maternal and perinatal morbidity.

The book discusses the immunologic, clinical and epidemiologic evidence for altered responses during pregnancy. Several infections have unique consequences in pregnancy. Some infections have vertical transmission, and their management focuses on decreasing perinatal transmission. Others can be transmitted transplacentally and cause congenital infection. While still, other common infections like gastroenteritis, UTI, tuberculosis, leprosy or certain dermatological and oral conditions can cause pregnancy complications. This book discusses all such diseases in detail as well as suggests meansfor early identification and appropriate treatment for them.

A separate chapter adequately covers the novel coronavirus infection associated with management challenges in pregnant women. The book includes dedicated sections on postpartum infections and fetal outcomes associated with maternal infections. It reviews strategies to prevent infection in obstetrics that plays a key role in decreasing the global burden of maternal morbidity and mortality.

About the Author

Dr Sumita Mehta is Senior Specialist and In-charge, Obstetrics & Gynecology at Babu Jagjivan Ram Memorial Hospital, Delhi. She graduated from Lady Hardinge Medical College and completed her postgraduate degree at Maulana Azad Medical College, Delhi University. She is a certified trainer for colposcopy for the state of Delhi as well as the International Agency for Research on Cancer (IARC) and was the secretary of the Indian Society of Colposcopy and Cervical Pathology (ISCCP). She is an executive member of National Association for Reproductive & Child Health Care of India (NARCHI) and has previously also served as an executive member and editor for the Association of Obstetricians & Gynecologists of Delhi. She has conducted several colposcopy workshops all over India and has delivered numerous lectures in various academic forums. She is the author of 9 books and has over 75 publications in national and international journals of repute. Her primary area of interest is high risk obstetrics and preventive oncology. Her peers hold her in high esteem for her academic and professional acumen.

Dr Anshul Grover is Specialist, Obstetrics & Gynecology at Babu Jagjivan Ram Memorial Hospital, Delhi. She has graduated from the esteemed Lady Hardinge Medical College and done her post-graduation from Maulana Azad Medical College, Delhi University. She has over 20 years’ experience in obstetrics. She has contributed chapters in various books and is also the author of many publications in national and international journals. She is an active member of national societies like FOGSI, AOGD, NARCHI, and ISCCP and has participated as organizing team and faculty in various conferences. She has conducted training of medical officers in Reproductive health under the Delhi Government. She has also held the post of joint secretary of the Indian Society of Colposcopy and Cervical Pathology (ISCCP) from 2016-2018.Her areas of interest are high risk obstetrics and adolescent health.

Description: This book offers a summary of major infections affecting pregnant women and their fetuses. It is divided into 10 parts or chapters, and these are further subdivided into three or more sections each, which cover individual or grouped infections or infectious diagnoses. It contains tables, figures, boxes, and several algorithms for diagnosis and treatment decision support. There is no index.
Purpose: The book describes epidemiology, pathophysiology, clinical manifestations, diagnosis, and treatment considerations for many common and some less common infections in pregnancy.
Audience: The book is tailored toward a global general OB-GYN readership and most guidance is based upon World Health Organization recommendations, although guidance from the U.S. and U.K. is referenced as well in some chapters. The content reaches its target audience.
Features: The review of some infections that are less common in developed nations, such as tuberculosis, would be welcomed by physicians less familiar with managing these infections that contribute to high maternal morbidity in developing countries. More references would be appropriate for some chapters. The chapters have, at times, complex organization and might benefit from additional editing. Some figures are less helpful for the generalist obstetrician-gynecologist. At times, figures and tables appear in unexpected places and could be better resources to the general OB-GYN if relocated to chapters oriented to the material contained in the figure. The book would benefit from more high-quality color images with clinical and ultrasound manifestations of maternal and fetal infections, respectively. In some chapters or sections, a discussion about the vaccine-preventable nature of some infections is lacking.
Assessment: This book provides an important overview of infections for a global OB-GYN audience, although it is less valuable as a resource for U.S. physicians compared with clinical practice guidance published by the American College of Obstetricians and Gynecologists, the Society for Maternal-Fetal Medicine, and the Centers for Disease Control and Prevention.
