Description: These guidelines are an update of the second 2017 edition. Aimed at supporting the ambulatory obstetric care setting clinician, these guidelines seek to condense the current evidence-based knowledge of preventive care, patient self-care, and the identification and management of antenatal and postpartum problems and pathologies including clinical manifestations, diagnoses, and treatments.
Purpose: The purpose of this book is to provide a quick reference guide to the key features in the clinical management of physiological processes as well as pregnancy and postpartum related problems and complications encountered in the ambulatory care setting. The provided guidelines have a strong focus on preventive care in addition to patient education and counseling.
Audience: The intended audience is nurse practitioners, nurse midwives, clinical nurse specialists, physician assistants, and other health professionals providing antenatal and postpartum care in outpatient care settings. Students within these clinical subspecialties are also included in the intended audience.
Features: The book provides guidelines in four sections: preparation for pregnancy, routine prenatal and postpartum care, common problems of pregnancy, and management of selected complications of pregnancy. Each of the four sections provides two to 10 guidelines for a total of 24 antepartum and postpartum care guidelines with the most detailed section covering complications of pregnancy including COVID-19. Six appendixes offer additional information on patient resources, screening tools, HIV testing in pregnancy, federal employment laws on work breaks, telehealth resources, and LGBTQIA+ resources. Guidelines are provided in outline format. All guidelines include background, history, physical examination, laboratory and diagnostic testing, patient education, health promotion, management, complications, consultation/referral, and follow-up. The guidelines include clear tables, figures, boxes, and complete current literature bibliographies.
Assessment: The clinical guidelines provided offer concise and clear descriptions of the safe clinical practice components. A well-educated clinician with some years of clinical experience can safely use the contents of this compact 350-page spiral bound tome. However, this book also provides an avenue for those who lack the didactic and clinical education as well as the patient care experience to potentially misuse the content in what could be clinically dangerous "cookbook" fashion. In the preface, the editors state that the book is aimed at clinicians caring "for low-to moderate risk pregnant individuals" (p. xxii). A key omission is the absence of a similar statement as to the non-optional imperative need, for the sake of maternal, fetal, and/or neonatal/infant safety, to consult, collaborate, and refer patients to other clinicians (e.g., OB-GYNs, perinatologists, endocrinologists) as needed in the care of moderate to high risk pregnant or postpartum individuals. An exemplar of this omission is found in the gestational diabetes mellitus (GDM) guideline in which, in the care of a patient requiring insulin for GDM management, the advice includes "comanage with physician or consider referral to physician if the patient requires insulin" (p. 242). For safe practice, the referral in the case of patient requiring insulin should not be represented as optional.
