Description: This book focuses on fetal cardiology from the unique perspectives of the pediatric cardiologist, obstetrician, and maternal-fetal medicine specialist. It includes an extensive review of the physiology of the placenta, structural malformations of the heart, 3D and 4D ultrasound evaluation of these malformations, and the pre- and postnatal management of cardiac malformations. This is the second edition of a book in the Series in Maternal-Fetal Medicine.
Purpose: The purpose is to provide a comprehensive guide in fetal organ scanning for anyone interested in the fetal heart. This book comprehensively reviews placental vascular and fetal cardiac normal development as well as dysfunction or maldevelopment of these systems.
Audience: Sonographers, residents and fellows in obstetrics, maternal-fetal medicine, and pediatric cardiology are the intended audience. It is also a valuable, comprehensive review for practicing physicians in these specialties.
Features: The book extensively reviews variations on cardiac anatomy, including the complete prenatal evaluation of each lesion and a discussion of postnatal management. This allows practitioners an opportunity to develop a better understanding of the implications of congenital heart disease both pre- and postnatally. Additionally, the book reviews placental vascular development and pathologies, fetal peripheral Doppler, twin-twin transfusion syndrome, and fetal arrhythmia. Finally, chapters focus on newer technology of 3D and 4D fetal cardiac imaging. This discussion is augmented by images on DVD. The topics of fetal cardiac imaging, Doppler, and rhythm assessment are all well covered. The chapters on various cardiac lesions are complete, although sometimes redundant. The chapters on 3D and 4D imaging are interesting, but not as useful to the practitioner. The DVD adds imaging loops and some excellent explanations of obtaining 3D and 4D images, but otherwise it adds little to the book. It cannot be searched chapter by chapter, and many of the image loops improve little on the excellent images in the book. The references are up to date and very thorough. The organization of the chapters is a bit awkward and occasionally repetitive.
Assessment: This is a high quality, comprehensive review of the evaluation of the fetus and placenta for pathology related to the fetal cardiovascular system. The images augment the text and result in an extremely useful guide. I found the discussion of 3D and 4D imaging, the touted reason for this second edition, to be of less use in my day-to-day practice in the evaluation of the fetal heart and more of an interesting summary of current technology. The DVD does not significantly enhance the text and has some minor technical issues (missing images for one chapter, single beat loops for some sections, inability to search by chapter or video image). Despite these shortcomings, this is a very useful book for those in the field of fetal imaging.
