Overview

Approximately 80 percent of the world's population is pigmented; even in the United States, current projections indicate that by mid-century the majority of the population will be non-Caucasian. US dermatologists are already seeing a significant shift in the makeup of their client population. Yet, despite this changing face of the American population, until now practicing dermatologists and students in training have been challenged by the lack of a comprehensive textbook on the diagnosis and treatment of pigmented skins.

Dermatology and Dermatological Therapy of Pigmented Skins fills a serious need by providing an exceptionally comprehensive text on the practice of dermatology as it applies to the variety of skins found in the U.S. population. As the editors note, its not simply about dark skin versus light skin. Blacks, Asians, Hispanics, Native Americans, and Slavs all have different skin profiles and consequently require different treatment. The presentation of common dermatologic disorders will often vary from one ethnic group to another, and while some disorders may be found frequently in some groups, in others they are rarely found. Furthermore, the application of medical and surgical therapies will sometimes require different parameters for differently colored skin.

Description: This book provides a good overview of skin problems commonly seen in people of ethnic backgrounds whose skin tends to have larger melanosomes and more active melanocytes. There are many color photographs, the quality of which varies. Some of the photographs are too dark or are taken too far away to see detail. Sound discussions of cellular physiology enable the reader to understand the mechanisms for particular changes that occur in pigmented skin.
Purpose: The purpose is to educate dermatologists about evaluating and treating pigmented skin.
Audience: The audience is dermatologists.
Features: All the topical treatments for postinflammatory hyperpigmentation and melasma are discussed, including their pros and cons. The discussion of cellular changes in the epidermis and dermis associated with photoaging in different ethnicities is well done. A more detailed explanation as to why black skin tends to form hypo/hyperpigmented annular plaques would have been appreciated; the authors state it is because the skin has a tendency for fibroplasias, but I was not sure how this contributed to an annular plaque versus a plaque. Suggested product names for a protective emollient on the scalp for Afro-Americans having hair straightening would be helpful. There are useful clinical tips for treating acne in darker skin patients. Those products which tend to be more irritating in these populations are discussed.
Assessment: The authors provide useful information to clinicians treating darker-skinned individuals. I like the authors' inclusion of Asians, African-Americans, and Hispanics in their presentation. Previous textbooks have concentrated on only one ethnic group. This book gives you a unified approach to all pigmented skin types.
