This is a comprehensive overview of the theoretical basis of cancer screening, the data available to support screening for specific cancers, and future developments in cancer screening. The book gives a balanced view of the complicated issues surrounding cancer screening including the limitations of screening, informed consent, and medical decision-making. The purpose as stated by the editors is to describe how one might translate what is known about a particular screening test into action and how to involve the person being considered for screening in the decision. While the first part of this aim is realized, and the importance of informed decision making is stressed, the book does not offer much insight into how to simply translate what is known about the complexities of the risks/benefits of screening procedures to persons considered for screening. The target audience includes medical and other health professional students, and physicians and healthcare professionals involved in advising individual patients and the public about screening. The book is written at an appropriate level for this audience. The book is organized into four sections: background, basic principles of screening, screening for specific cancers, and future developments. In the first two sections the editors explore the emergence of cancer screening from an historical perspective, review the basic concepts of screening (including issues of the ethics of screening, consent, and decision making), and discuss the regulation of medical devices by the Food and Drug Administration. The discussion of screening theory and study design is particularly well written and useful. The third section addresses the screeningof specific cancers: screening tests available, evidence to support their use, controversies, and recommendations from professional organizations. These chapters are an excellent up-to-date resource for cancer screening of the individual cancers, with current, pertinent references. While the authors of these chapters summarize the available information and recommendations on cancer specific screening, they do not uniformly provide their own recommendations for screening. The book concludes with a discussion of new screening technologies and the implications of genetic inheritance of cancer risk for cancer screening. This is a very useful and informative book. It provides a balanced view of the major issues and controversies in cancer screening, and offers comprehensive coverage of screening for specific cancers. Unlike the U.S. Preventative Service Task Force's Guide to Clinical Preventive Services, 2nd Edition (Lippincott Williams & Wilkins, 1996), this book does not provide recommendations for specific cancer-site screening but instead provides the reader with extensive, useful information for judging the utility of and making decisions about screening tests.
