Overview

Basic research on osteoarthritis has been carried out mainly from the his­ tological and biochemical aspects of the degenerating chondrocytes, collagen fibers, and matrix proteoglycans. Undue mechanical stress has been shown to be the principal factor in the initiation of osteoarthritis. Although the exact process by which mechanical stress leads to the total destruction of cartilage tissue remains unclear, several new research methods have enabled us to gain a deeper understanding of the process of degeneration. In October 1997, we organized an international symposium titled "Ad­ vances in Osteoarthritis" in Kobe, with the main topics being updated research, diagnosis, and treatment of osteoarthritis. The proceedings of the symposium are presented here in five sections: (1) Mechanical stress and reactions of chondrocytes, such as intracellular ion changes, changes in the cytoskeleton, intracellular messenger systems, release of gas mediators, and changes in electromechanical properties of cartilage; (2) Functional diagnosis of osteoarthritis by MR imaging, and using calpain and collagenase III as new cartilage markers; (3) Treatment with a promising simple washout technique and IL-1RA and MMP antagonists; (4) Cartilage repair by new grafting techniques; and (5) Problems following total joint replacement. We sincerely hope that the advanced knowledge provided in this volume of proceedings will be valuable to our readers.

Description: This is a compilation of the recent scientific advances in osteoarthritis as presented at an International Symposium in Kobe, Japan in October, 1997.
Purpose: The purpose is to update the state-of-the-art as it relates to research, diagnosis, and treatment of osteoarthritis. These objectives are more than adequately met by the contributors. The editors are from the Department of Orthopedic Surgery, Kinki University School of Medicine, in Osaka, Japan. They did an outstanding job in putting this very important symposium into print. However, very little information is given about the other contributors.
Audience: This book is written mainly for orthopedic surgeons and researchers of the musculoskeletal system. It is written at the appropriate level.
Features: It is broken down into six sections: Mechanical Stress and Chondrocyte Reactions; Functional Diagnosis of Osteoarthritis; New Cartilage Markers; Promising Treatments 1; Promising Treatments 2 — Cartilage Repair; and Problems Following Total Joint Replacement. The summary and conclusions of the chapters are most helpful for readers who are not interested in the small details. The key word index is brief but might be appropriate for this type of textbook.
Assessment: Articular cartilage is a very exciting area in musculoskeletal research. The replacement of a worn down articular cartilage joint by total joint replacement is the major contribution of arthritis treatment in the last thirty years. Trying to prevent or delay cartilage wear and the development of cartilage engineering tools are the next major endeavors for the next millennium. This important work is most timely. It is recommended to all medical professionals interested in arthritis. Bookstores are advised to stock this work.
