Description: This is a collection of vignettes designed to illustrate the management of common clinical conditions featuring the use of critical care and acute surgical therapies.
Purpose: Management of clinical problems is placed in the context of a clinical presentation with sequential questions leading readers through care of the patient.
Audience: Senior students and junior trainees are an appropriate audience for this work, written by providers in North American trauma and critical care programs.
Features: There is no particular order to how the 19 patient cases are presented. For example, the initial chapter describes management of intracranial problems such as hypertension and status epilepticus, while the next chapter describes conflicts in ICU care. A variety of common problems are discussed including liver failure, quality concerns in the critical care unit, sepsis and septic shock, brain death, nutrition support, airway management, treatment of bleeding, and problems in mechanical ventilation. Each chapter begins with a case presentation. In some chapters, multiple cases of a particular type are discussed. The chapters present a portion of the presentation with clinical data, followed by a question related to management. The rationale for care of the patient is then explained and the next steps in management are revealed. The chapters end with a reference list emphasizing recent important papers. References date to within two to three years of publication. Illustrations are infrequent, and tables and simple line drawings reproduce well. Case presentations are designed to feed readers laboratory data and important aspects of physical examination. Thus, the flow of each presentation is rapid. The table of contents lists chapters by title and acknowledges authorship. A brief subject index provides adequate access to content.
Assessment: This is a comfortable, readable, stepwise presentation of important administrative issues and clinical problems. While this book is accessible to readers at all levels of sophistication, many topics are not covered. For example, essentially no cardiac critical care is described. Thus, this book is best used when it complements a larger, more expansive book.
