Description: This is a multiauthored book with very fragmented chapter structure and overlapping content addressing "acute" rheumatic diseases.
Purpose: There is not a compelling need for a compendium of critical care management problems in rheumatic and immunologic diseases. Critical care management for organ failure "per se" is best described in texts of intensive care medicine. The rare peculiarities and unique needs of the critically ill rheumatologic patient can be found in current rheumatology texts.
Audience: The intended audience is difficult to define but appears to be primary care specialists and medicine/pediatric house staff.
Features: The book has no unique features and suffers from an overuse of algorithms printed in a minute font. Many experienced rheumatologists would take exception to recommended treatment and diagnostic paths proposed, e.g., those for fever and arthritis.
Assessment: The book gets off to a rocky start because it is badly mistitled. Most of the chapters actually deal with acute or chronic manifestations of chronic rheumatic diseases, and many of the management problems discussed do not qualify as critically ill episodes. Moreover, any book that begins with a chapter on laboratory evaluation of a patient, before clinical assessment, is probably headed in the wrong direction. There are also striking deficiencies in content. For example: allopurinol-azathioprine potential fatal interactions in transplant settings are not even discussed, vasculitis is classified in the obsolescent framework of primary versus secondary, and ANCA classification of these syndromes is not even mentioned. The cyclosporine induction dosage noted is 2 to 3 times higher than the one actually used. The reader is better served by referring to several excellent critical care and rheumatology texts currently available.
