Overview

Acute organ damage and the ensuing multiple organ failure are the result of a pathophysiological process involving various cytokines. Once activated, these proteins cannot be eliminated even when the kidneys function at their maximum capacity. To counteract this mechanism, researchers in Japan have developed an innovative concept employing blood purification to remove the overwhelming cytokines.This book describes the use of hemodiafiltration to inhibit the cytokine storms which cause serious organ damage in patients with septic shock. Purpose: The book reviews blood filtration technologies that provide support for renal and hepatic insufficiency, sepsis, and other problems.
Audience: Intensivists and nephrologists with an interest in advanced applications of renal replacement and blood purification technology are an appropriate audience.
Features: After an overview of applications for blood purification technology and terminology, the book discusses the use of blood filtration first for renal failure associated with sepsis and nonseptic etiologies. Later chapters review blood purification in the setting of pancreatitis, fulminant hepatic failure, ARDS, and intoxication states. The book details individual applications, then reviews technologies under development specific to blood purification in these applications. Concluding chapters describe selection criteria for specific blood purification technologies. Chapters are clearly written, and include abstracts and summary points. Line drawings and other illustrations are effectively used and reproduce with good quality. Each chapter has an appropriate reference list, typically 5 to 20 citations, dating to the year of publication of the book, and including original studies.
Assessment: The Japanese have long been leaders in the expansion of dialysis technology to broader roles in blood purification for critical illness. This monograph provides an excellent introduction to this work without burdening readers with technical details of each application. I recommend this work to intensivists with an interest in new horizons for blood purification in the ICU.
